Notification of Moving Out

If you will be leaving the country for more than a year or if you will be moving to an address outside of Saga City please file a Notification of Moving Out in advance.

Things to Bring When Filing your Notification

○ Documents verifying your identity (Resident Card, Special Permanent Resident Certificate, Items considered as Resident Cards, Drivers License, Health Insurance Certificate, etc.)

※If a proxy will be doing the procedures on your behalf, a Power of Attorney Form is required. In addition the proxy must bring documents verifying their identity (such as a driver’s license.)

※Once you have finished move out procedures for Saga City you will receive a Change of Address Certificate. Please bring this with you and use it during move in procedures at your new address.
