National Health Insurance
|
|
The National Health Insurance Plan is a public medical insurance program in
which all people in residing in Japan are enrolled. This National Insurance
Plan covers 70% of your medical costs if you are injured or otherwise become
ill.
Everyone who has undergone foreign resident registration, and has a visa for
a period of stay longer than one year, and is not enrolled in any other
public health insurance system is REQUIRED to enroll in the National Health
Insurance Plan.
To enroll please go to the municipal headquarters of the city, town or
village that you reside in. If you are a Saga city resident, please go to
Saga City Hall, National Health Insurance and Pension Section, window number
7. To enroll you need the following:
a) Alien Registration card
b) Passport
c) Inkan (personal seal)
d) Bank account pass book
Only the head of the household need register, but please have all personal
information available for all members of the family when applying.
Once you have applied you will be issued an insurance card, one covers the
entire household. Please take good care of this. When going for medical
treatment always bring your National Health Insurance Card with you.
Leaving Japan- You are required to withdraw from the National Heath
Insurance Plan when leaving Japan permentatly, or if you move to another
municipality, or your period of stay has expired. File withdrawal forms at
the place where you enrolled in the plan.
Premiums for the National Health Insurance Plan are based on income. Please
inquire at the National Health Insurance and Pension Section of the City
Hall for details.
Be Aware, the following medical treatments are not covered by National
Health Insurance Plan
a) Heath Diagnosis and group health examinations
b) Vaccinations
c) Cosmetic Surgery
d) Normal pregnancy and childbirth **(live births, including stillbirths or
miss carragies after 85 days or more, of National Health Insurance Members
will be reimbursed 300,000 yen for each child born)**
e) Abortion or contraception induced for financial reasons
f) Treatment received outside of Japan
The following are excluded from treatment under the insurance plan:
a) Injury due to an illegal act, fighting, or drunkenness
b) Self-inflicted illness or injury |
